Risk assessment form

TTOS § 13 lõige 1

A structured risk assessment document for identifying, evaluating, and recording workplace hazards.
Mandatory for every employer, regardless of company size.

Employee notification record

TTOS § 13³ lõige 1

A record signed by the employee confirming they have been informed of health and safety risks.
The first document requested by the Labour Inspectorate during an inspection.

  • Control measures explained to the employee

Health surveillance checklist

TTOS § 13¹

A step-by-step checklist for meeting all obligations relating to health surveillance.
Covers the entire process from the service contract through to retaining decisions.

Occupational health situation analysis questionnaire

TTOS § 13 - mandatory from 01.01.2023

The mandatory 3-year summary submitted to the occupational health physician.
Covers hazard factors, health surveillance results, workplace accidents, and recommendations.

Workplace accident investigation record

TTOS § 24

Employers are required to investigate and document every workplace accident.
The record sets out the circumstances, causes, and preventive measures.
The Labour Inspectorate will request this document when investigating an accident.

  • Injured person's details (name, personal ID, job title, length of service)
  • Description of the accident (date, time, location, what happened)

First aid arrangements document

TTOS § 13²

Employers must ensure that first aid is organised at every workplace.
This document records trained first-aiders and the location of supplies.
A regular inspection schedule helps ensure supplies remain in order at all times.

  • List of first-aiders (name, training date, certificate validity)
  • Location of first aid supplies (building, floor, room)
  • List of supplies (bandages, plasters, defibrillator, etc.)

Working environment representative meeting minutes

TTOS §§ 17-18

Employee representatives must be able to participate in the organisation of the working environment.
The minutes document decisions taken and next steps agreed at each meeting.
Draw up minutes after every meeting and retain them for at least 3 years.

  • Signatures of the minute-taker and the representative

Hazardous chemicals register

Chemicals Act + TTOS

All hazardous chemicals used at the workplace must be registered.
A safety data sheet must be available for each chemical.
The register enables swift action in an emergency.

  • Safety data sheet number and date of last update

Employer's health and safety instruction for employees

TTOS § 13³

Every employee must receive written health and safety instructions before working independently.
The instruction covers hazard factors, personal protective equipment, and emergency procedures.
The employee confirms with a signature that they have read the instruction.

Health surveillance referral letter

TTOS § 13¹

Employers are required to refer employees for periodic health surveillance.
The referral letter gives the physician the information needed about workplace hazard factors.
Arranging health surveillance without referral letters constitutes a breach by the employer.

Frequently asked questions about the documents

Are these documents legally required?

The law does not prescribe a particular format - what matters is the content. These documents cover the elements required by the TTOS. Your occupational health service provider may have their own versions.

What is the most convenient way to use the documents?

Open the document in a new tab and use your browser's print function (Ctrl+P / Cmd+P). Select "Save as PDF" - you can then apply a digital signature using Dokobit or similar software.

How long must documents be retained?

Risk assessments must be retained for 55 years (TTOS § 13-4 lg 9; working environment data register § 24-1 lg 4 p 5). The occupational health service provider must retain health surveillance data for 30 years and the employer must retain service-arrangement data for 10 years (TTOS § 13-1 lg 16). Data on exposure to carcinogens or mutagens must be retained by the employer for 40 years; for reproductive toxicants, 5 years (TTOS § 13-1 lg 17).

When must the 3-year analysis be submitted?

The obligation has applied since 1 January 2023. If you have not yet prepared it, do so at the earliest opportunity and submit it to your occupational health physician. Delay constitutes a breach.
